Before Your Visit
To increase the efficiency of your visit, our office may mail you a new patient packet to fill out in advance or you may click on the link below to download and print.
Bringing the completed packet, your insurance cards, a list of current medications, discs with pertinent CT or MRI imaging studies and any other relevant medical records will help minimize any delay on the day of your visit. We will collect office co-pays at the time of the visit, as well as any portion of your insurance deductible that has not been met. Cosmetic or non-insurance related visits usually have a consultation fee of $200, which will be used as credit towards any treatment.
